A provider must submit an initial onboarding form and receive approval to participate in the program prior to referring clients. Eligible providers include any medical or behavioral health licensed professional.
Providers can refer patients via the BASE (Bridging Appalachian Systems Equitably) program funded by CareFirst via the Garrett County Health Department.
